Please read the following carefully. These are the terms and conditions agreed to upon purchase of class passes/memberships or other packages. 

  • I agree to complete a PARQ+ Physical Activity Readiness Questionnaire (sent via email OR on the website: www.vitalityhealthandfitness.org) and return a paper copy or digital copy via email before participating. I will update the information for this questionnaire as needed and inform trainers and instructors of any changes to my health and physical activity readiness.

  • I will provide an emergency contact in my intake information- someone whom a trainer can contact if an incident occurs at the studio

  • I understand that accidents can occur and it is my responsibility to ensure I am working safely in the space, with equipment and around others. This requires mindful transitions at all times.

  • I understand that Vitality Health & Fitness takes all necessary precautions to avoid accidents and should an accident occur Vitality Health & Fitness and 127 Queen St. Inc. is not responsible

  • I understand that if I started with a "12 Weeks of VItality" package it will auto-renew unless I otherwise cancel in a reminder email of auto-renew that I will receive 7 days before the auto-renewal date

  • I understand that all MEMBERSHIPS (not class passes) will auto-renew monthly from my start-up date, but that I have the option to cancel my monthly membership in a 7 day window leading up to auto-renewal.

  • I understand that my membership begins on the day of purchase.

  • I understand that the number of classes on my monthly membership must booked before my auto-renewal date.

  • I understand that unused classes do not roll over to the next month.

  • I understand that new classes are loaded onto my account at the time of auto-renew to start booking classes for the month ahead.

  • I understand that If I wish to cancel or change my auto-renewal I must do so within the 7-day window leading up to my auto-renew date. (A reminder email will be sent 7 days before auto-renewal with the option to cancel) If I choose to cancel my auto-renewal outside of the 7-day cancellation window a $40 fee will be charged for payment processing fees.

  • I understand that a 10-Class Pass can be used for any classes but must be used within 120 days of purchase.

  • I understand that Vitality Health & Fitness does their best to keep a consistent schedule but some classes are subject to change with the release of a month-to-month calendar.

  • I understand that when classes are needed to be canceled I will receive a text msg and email notification notifying me of the canceled class.

  • I understand that a minimum of 2 participants must be registered for a class to run. Otherwise, the class is subject to cancelation.

  • I understand that if I am a "no-show" to a class that I will be charged on my account for the "no-show" class. I know that I can schedule classes up to 1 hour before the class and I must cancel before 1 hour of the class.

  • I agree to photo/video consent at Vitality Health & Fitness unless I otherwise specify
